Power of Attorney

Thank you for visiting the Minnesota legal assistance office website. We look forward to helping you with your Power of Attorney.  In order for us to best serve you we have a couple of things we would like you to complete. 

1. Complete each part of the Client Card when you send in your Power of Attorney information.

2. Read the Power of Attorney (POA) Questionnaire.

3. Complete POA Questionnaire, fully detailed.  Answer every question, even if answer is negative.

4. Send the client card and POA Questionnaire to the legal assistance office. A paralegal will confirm it is complete or return it to you for completion.

5. When PAO Questionnaire is complete, your POA will be completed and sent to you for review.  If it is correct you can take it to a notary locally or set up a time to come in and get it notarized in the legal office.
