Dental
Veterans may receive one-time dental treatment if they were not provided treatment within 90 days of separation from active duty. The time limit does not apply to veterans with dental conditions resulting form service-connected wounds or service injuries
For current members, the following form is provided to support dental readiness. Members should provide their civilian dental provider with the Dental-Civilian Provider Form and ask them to forward the document to the service member's National Guard unit.
For AGR Soldiers, in addition to the Dental-Civilian Provider Form, the member should also provide a completed copy of the Military Medical Support Office (MMSO) authorization form to their civilian dental provider.
