To request personnel records, a written request with signature is required. Please download the Standard Form 180 (Acrobat PDF Reader is required.) Follow the instructions provided with the form. Mail the form to:

Military veterans and the next of kin of deceased former military members may now use a new online military personnel records system to request documents. This may be particularly helpful when a veteran needs a copy of his DD-214 for employment purposes. Veterans Records online are found at http://www.archives.gov/veterans/military-service-records
